Medical emergency? Call 911 or go to the nearest emergency room. Please don't use online booking or messages for urgent problems.
Before your visit
Please bring:
- Photo ID and your insurance card
- A list of your current medications and doses
- Recent records or test results, if you have them
- For children: vaccination records
- For DOT exams: driver's license, plus glasses or hearing aids if you use them
If it's your first visit, please arrive 10–15 minutes early to fill out paperwork. We're on the first floor, with free parking right in front.
