What the exam checks

  • Vision: at least 20/40 in each eye, with glasses or contacts if you use them
  • Hearing: hearing a whispered voice from 5 feet away, or a hearing test
  • Blood pressure and pulse
  • Urine test: checks protein, blood, and sugar for health conditions. It is not a drug test.
  • A physical exam and a review of your health history and medications

The forms

The exam uses the official FMCSA Medical Examination Report form (MCSA-5875). You fill out Section 1: your contact details and a health history questionnaire about past surgeries, medications, and conditions such as diabetes, heart disease, or sleep apnea. Answer fully; the examiner reviews your answers with you. You can download the form from FMCSA and fill out Section 1 before your visit, or arrive 10 minutes early to fill it out here.

If you meet the standards, you receive the Medical Examiner's Certificate (MCSA-5876), and your results are reported to the FMCSA National Registry.

What to bring

  • Your driver's license
  • Glasses, contact lenses, or hearing aids, if you use them
  • A list of your medications and past surgeries
  • If you have a condition such as diabetes, heart disease, or sleep apnea: recent records from the provider who treats it (for sleep apnea, your CPAP usage report)

DOT physical vs. a regular physical

A regular physical looks at your overall health and prevention. A DOT physical checks whether you meet the federal safety standards to drive a commercial vehicle, and only a certified medical examiner listed on the FMCSA National Registry can do it. It does not replace your yearly checkup.

Can I get a DOT physical online?

No. The vision, hearing, blood pressure, and physical exam have to be done in person.

How long is the certificate valid?

Up to 2 years. If you have a condition that needs monitoring, such as high blood pressure, the examiner may certify you for a shorter time, such as 3 months, 6 months, or 1 year.
