  • Diagnoses and a list of chronic conditions
  • Recent lab results, ECGs, and imaging reports
  • Hospital discharge summaries and surgery reports
  • A list of medications, including the generic (chemical) names
  • Vaccination records, especially for children, who need them for school

Do my records need to be translated?

Our provider, Natalia Pasumansky, DNP, speaks Russian and Hebrew as well as English, so bring records in those languages as they are. For records in other languages, a translation of the main diagnoses and medications is helpful.

Medication names abroad

The same medicine often has a different brand name in another country. Bring the packaging or write down the active ingredient, so your provider can find the US equivalent.
