Many schools, sports teams, and camps ask for a recent physical exam. A yearly well-child visit is also the best time to check growth, development, and vaccines, and to ask any questions you have.
What happens at the visit
- Height, weight, and blood pressure
- Vision and hearing check
- A physical exam, including heart and lungs
- A review of vaccines and any that are due
- Questions about sleep, nutrition, school, and mood
What to bring
- Any forms from the school, team, or camp, filled in with your part
- Your child's vaccination record, especially if you moved from another state or country
- A list of medicines and allergies
- Glasses or contacts, if your child uses them
Vaccines and school in Washington
Washington schools and child care centers ask for a Certificate of Immunization Status that shows your child's vaccines. If your child's records are from another country, bring them to the visit and we will review them with you. Childhood vaccines are available at our clinic.
Teens
As children get older, the yearly visit is also a time to talk about sleep, screens, stress, and mood. It helps teens learn to take care of their own health, and you are always welcome to share your questions too.
Tips
- Book early. Late summer and early fall are busy, so schedule a few weeks before forms are due.
- Sports physical or full checkup? A sports form covers what a coach needs. A full well-child visit covers your child's overall health too, and it can include the sports form.
- Records from abroad: bring them as they are. Our provider reads Russian and Hebrew.
